| 内容記述 | 本研究では、訪問介護事業所のサービス提供責任者による訪問看護事業所の訪問看護職や居宅介護支援事業所の介護支援専門員とのチーム活動（以下、「チーム活動」）とサービス提供責任者がチーム活動を行う際のチームメンバーに関する情報の把握との関連を検証した。調査対象および方法は、A圏でwamnetに登録されており、無作為で抽出された訪問介護事業所のサービス提供責任者500名であった。各事業所に1名、調査票を郵送し返送してもらった。分析対象は256名（51.2%）であった。分析の結果、「チーム活動」の全因子（「支援の質を高めるためのチーム活動」「支援を円滑にするためのチーム活動」の2因子）において「チームメンバーに関する情報入手」（チーム活動を行っている関係事業所の情報、医療的ケアにおける介護職の業務制限等、チームメンバーの役割と責任に関する知識、在宅高齢者および家族が最も信頼しているチームメンバー）か関連していた。 | |||||

| 内容記述 | People in charge of care services provided in home-visit care service centers become involved in team activities with home-visit care from these centers and care managers from bome-visit care support centers. The present study examined the relationship between these team activities and the acquisition of information on other team members by home-visit care providers. The subjects were 500 randomly selected care providers working in home-visit care service centers registered in WAM NET in Area A. Questionnaires were distributed to one care provider from each center, and they returned the completed forms by mail. A total of 256 care providers (51.2%) were analyzed. As the results of analysis, two team activity-related factors ("team activities to improve the quality of support" and "team activities to facilitate support") were closely associated with "the acquisition of information on members involved in team activities", including information on relevant centers involved in team activities, restrictions on tasks that can be performaed by home-visit care providers, knowledge of the roles of team members and their responsibilities, and information regarding team members most trusted by the elderly living at home and their families. | |||||
